In the event of a medical emergency involving our child, we give permission to Montgomery Academy and to any of its personnel to obtain the services of a physician or available hospital. We understand that we are responsible to pay all bills for any medical attention Montgomery Academy obtains for our child. This includes hospital bills, doctor bills and all other costs (such as laboratory, x-ray and other tests) arising from medical attention Montgomery Academy obtains for our child.
